"Error: Initializing Form" - Survey123 in Browser

Question

I have a survey published from Survey123 Connect.

It opens fine in the Survey123 application, but when one tries to open it online in a browser the following error occurs.

I've also attached the XLSX form the survey was published from.

Has anyone else had similar issues? If so, any suggestions on how to fix?

I'm currently experiencing the same thing on a survey that has been functioning fine all year up until today. Any suggestions?

I wonder if it doesn't have something to do with the commas and quotes in your choice list names.

I removed the commas and quotes in your choices and also the commas in your survey name and republished and it seems fine on Survey123 Web:
