I'm creating a S123 on hosted layers. As guided, I set the Form_ID and submission_url in setting. But I got error like "The custom feature service submission_url is not compatible with this survey. Request error Missing serviceItemID"
My submission_url format is like "https://psgx.canadaeast.cloudapp.azure.com/portal/sharing/rest/content/users/data_updates/items/fe9b1b4a45a84777b3eaaab574b46add"
It's on portal and under user "data_updates" sub folder. Any clue? Thanks!

I would do it the other way - in Connect, choose to create a new survey and then choose "Feature Service" - the submission url and form id settings will be populated for you.
