Good morning. We are designing a SUrvey form to get information about problems in some communities. The end user asks us to only place the location map on the form, however, to carry out subsequent analyzes and Dashboard with indicators, it is necessary to know to which State, municipality and parish that element belongs. We ask if it is possible through Survey using only the map, to append to the database the information about the Name of each of these territorial units.

Yes this is possible. See documentation link here on reverse geocoding in Survey123 using the pulldata function. If the state/ municipality/ parish don't need to be added until data collection is complete, you could also bring the point layer of survey responses into ArcGIS Pro or the ArcGIS Online Map Viewer and perform spatial joins with state, municipality, and parish layers to append those fields to the survey responses.
