Seems like the App does not work with secure services?
getting error on feature submit:

What is your email Erwin?
After further testing determined that the token is not the issue for me.
seems like App works, but if I enable attachments the submitting fails.

Quick Report supports secured feature service. This is an example of a secured feature layer
Can you try if this works on your end?
Is it possible for you to share the feature service URL and create a dummy account for us to test? If not, maybe you can try working with Esri Technical Support to troubleshoot this issue further.
