Unable to import shapefile onto ArcOnline

I am trying to import a zipped shapefile onto ArcOnline and keep getting this error "Unable to import this shapefile".

The most common issue I've run into (personally and with others) is #1c in the referenced article - it's easy to end up with a shapefile in a folder, and that small oversight will trigger the generic error when uploading to ArcGIS Online.
