How to solve Weighted Overlay Service warning (no title, no description, etc) when already configured?

Question

Hi, I was trying to add a weighted overlay service for modeler in GeoPlanner. I have downloaded the latest weighted overlay toolbox and configured each layer to include description, title, range values, etc as described in the tutorial. But every time I added a new model, a warning appeared (in attachment). Does anyone have an idea how to solve this?

Hi Lusi,

Would you be able to provide a screen shot of your mosaic dataset's attribute table?  Would like to see what values you have populated for each of these fields.
