Error Initializing form,failed to load data form

Hello,

I am having trouble accessing my survey on the web. I think the problem is linked to my pull data function. 

I can access the survey from the phone app but whenever I try loading the survey on the web I get a message box saying "Error initializing form, failed to load data form".

Anyone has had the same problem ? I made sure there were no special characters in my column name and I also removed all extra columns for the csv file, but I still get the error message.

Hi Molina,

The issue has to do with the capitalization of 'SprayInfo' matching the filename exactly.  On lines 14 & 15, you refer to it as 'Sprayinfo' (note the lowercase i), which causes the error.
