Why don't I have any icons or buttons in Survey123 for ArcGIS?

No buttons or icons for Survey123 makes it pretty worthless. Especially annoying after spending time developing a form.

For all of us government employees, who are stuck with Esri and Microsoft, please ensure compatibility with Internet Explorer 11 before releasing. THanks!

Hi Steven,

It appears you are using groups and the groups are being loaded as collapsed by default (minimal appearance). This can be seen by the arrow on the left hand side in your screenshot. Selecting this arrow will un-collapse the group and show all the questions inside it.
