PullData (.csv) error on WebForm

I published a survey using  S123 Connect (v3.1.126).  Requirement is to use webform however the survey does not workvia the web app.   It will work in the native application.

Getting error

Made sure to use the concat() function for calculating the geopoint location in the form.

Everything looks to be correct .Even have other forms that are formatted similar and working.  Kind of stuck here.  Please help.

Hi Ian, thanks for providing the ZIP file. Looks like the space immediately after pulldata is tripping things up. I've tested it with the space removed, i.e. pulldata('mylocations', 'lat', 'name', ${dispatch_location}) and the location in the web form now appears to be pulled correctly. Please see XLSX attached.
