Fail to draw map tiles !

Question

This is the error that I get on my web mapping app

Failed to load resource: the server responded with a status of 404 (Not Found)

http://gis.collincountytx.gov/electionpollingplace/

This is a web server that is hosting the app inside the DMZ requesting the ArcGIS Server (two of them behind a load balance F5) tiles from a different location/server inside the network

...and this is the ArcGIS Server that is hosting the maps

Did you find a solution? I currently have the same problem if I use the web adaptor I have empty tiles, but if I don't use (directly to the 6080 port) everything works fine

There appears to be an issue with the map tile creation process. As soon as the map zooms to level 13 there are missing tiles. I would look into recreating the tiles with Manage Map Server Cache Tiles tool and use the RECREATE_EMPTY_TILES update mode.
