Preparing map layout templates

I would like to publish my own layout templates to print content from a web map. I found good information regarding how to publish them as a service in ArcGIS for Server(Tutorial: Publishing additional services for printing—Documentation (10.3 and 10.3.1) | ArcGIS for Server ), however I would like to know what are the steps needed to create a layout template. Please let me know if any of you can help me with this issue.
