How do I add agency logo customize the appearance in Survey123?

Question

I can see that there are several ways to customize the survey form but I am really just trying to add the DC Department of Health new logo on the form, possibly in the header or footer only.  Is this possible?  From what I have seen so far, it looks like I have to have a urL?  I really just want to cut and past the logo onto the form if possible but I cannot as yet seem to be able to accomplish this.

Hola,

what i did was copy my institution logo into the folder media, that is saved in the C directory, you have to find the folder named my survey designs then right click over the image properties where appears the media name, just select the name, copy and paste in the media::image column into your xcelform, and it has to work, you will be able to see your logo in the botton of your form.
