Can you please provide more details? e.g. what version of Survey123 Connect, are we re-publishing an existing survey, what changes have we made to the XLSForm?
Is it possible to provide the XLSForm?
Generally I would anticipate that this error is caused by invalid XLSForm configuration. Missing elements or overwritten formatting.

The issue is because you have three questions, with Names:
The 'instance_name' in the settings sheet is referencing a question with name 'example1'
As 'example1' does not exist, it throws an error during construction.
Also just noticed you've posted this in Python. Please move the thread to the Survey123 community for more visibility.
