How do I connect a web experience builder app into Survey123?

I know most people say to embed the survey into the web app builder but I want to do it the other way around. This is for the emergency response solution, and I want this because we have a small staff so we might not have the option to add the emergency response points on the map side but I want to give our information person the option to add it on the map so that they can send it out all together. ArcGIS Enterprise 11.5

If that is the case, you shouldn't need to embed the ExB to collect the location, you should just be able to collect a geopoint in the form. Then it should show on the map a long with all the info collected by the form.

I believe so. It talks to the same dashboard.

Is the form using the same feature service source as the ExB map?
