Surveys won't submit for one survey user

Hi I'm a bit of a novice, but we have a survey form that has stop allowing a one person in the team to submit their survey forms.  It also doesn't allow other users to submit forms with their name selected either.  this has previously worked so I'm a little confused of why this is now happening. I'd welcome any thoughts or things to try for this.  Thank you

You don't have to have them enter/pick their names. There's a question type called username that will automatically pull their login name. There's a similar question type for email. You can also use pulldata('@property', 'firstname'), or last name, to retrieve those. You can hide those fields and/or make them read only.

Are the names a choice field or are you using pulldata to get the name associated with the account?

If it is a choice field how are the names associated with the question? Are they in the choices of the survey, a csv file, another layer?

If it is pulldata, does the person's account still have the correct license and/or access to the survey?
