is there an Instant App to show a map, popups, and layer(s) attribute table with the option to toggle layers on and off on the map?
Table is available in Sidebar via the Layer list configuration. Once you've enabled the layer list you can choose layers from the dropdown then enable add table.
The current process can be a little confusing so in the upcoming release of Online you'll no longer need to select a layer you just choose to enable tables for layers in the layer list
There's a good documentation page, and the Instant App builder itself does a great job of previewing the different templates. I'd encourage you to open it up and explore them - noting any updates they have flagged - so you know all the templates available. There's some good ideas in there.

the Sidebar doc does say that sidebar support attribute tables but i think the functionality is not there anymore
