The Survey123 community gallery in Survey123 Connect and the Survey123 web designer showcases location-based templates for creating powerful data collection forms. In this blog, we will encourage you to contribute your own survey designs to enrich the community's experience, guide you through the exciting features of our community gallery, and showcase some remarkable templates created by users like you.
Share your survey designs with the community
We want you to think back to some of the amazing work you’ve done and that you believe others could use at their organizations. It doesn’t have to be a complex survey, but those are welcome too.
Got a survey in mind? Great! Now head over to the ArcGIS Survey123 community page to learn more about preparing your form for submission and when you are ready, click Submit your form. You can also access the page via the Contribute template button located in the web designer’s community template gallery. We can’t wait to see what you share with us.
Complete the submission form with all necessary details. Esri will review your survey designs and may offer suggestions. Upon approval, your survey design will be added to the community gallery in Survey123 Connect or the Survey123 web designer for others to use.

Survey123 web designer
To start exploring community templates in the web designer, click New survey and click Get started on the Template survey card.
In the Community section, you'll find a selection of user-created templates, categorized for easy browsing.
After selecting the desired template, click Use this template to quickly copy it into a new form in the web designer, ready for your customization.
Survey123 Connect
Ready to create a survey from a community template in Connect? Click New survey and then select Community in the side panel. Browse the selection of existing templates on various topics. Once you find one that works for you, click Create survey to start customizing it and make it your own.

This is a well-designed survey in German and a user-friendly tool designed for children and young people to assess urban green spaces from their perspective. This template enables them to provide valuable feedback that can be shared with urban planners to create green spaces tailored to their needs.
This bilingual template is designed for reporting an observed event related to the improper operation of water and sewage devices. Users can easily switch between English and Polish while filling out the survey. If you want to create a survey for a multilingual audience, this template is a great starting point.
Contributed by the Broward County Resilient Environment Department, this template serves as a citizen science crowdsourcing initiative designed to gain a deeper understanding of evolving flood patterns. This template offers multilingual support in English, French, Haitian Creole, Portuguese, and Spanish. This template is based on the US Army Corps of Engineers' Wetland Determination Data Form for the Atlantic and Gulf Coastal Plain (AGCP) region. Easily document wetland presence, including key indicators like hydrology, vegetation, and soil conditions. It's a digital alternative to the manual paperwork originally required.
