Feature reports not working

Good day,

I have published my survey on ArcGIS Online and when trying to generate the reports from survey123 website, the error message below appears, I used connect version 3.18.123

An error occurred when rendering by the report engine. Failed to print map for ${shapes}. Failed to load web map: c5b4c074bbda4cbc81719f1f42134fb7. Invalid URL: https://services1.arcgis.com/aiigQkyAFLnUvOAp/arcgis/rest/services/service_3bad31d4de034a15a97cbf545701d122/FeatureServer/0.

Yes, there is a repeat question in the survey as well as a pull data question from the web service.

The map's ID (c5b4c074bbda4cbc81719f1f42134fb7) isn't valid.

Can you locate that id on your Portal?

Also, can you toss in a different ID (e.g., b834a68d7a484c5fb473d4ba90d35e71) and does it work?

I have a form, when I want to see the data, it only loads in the second tab but nothing appears in the first. What could be happening?
