When I look at survey information which has been collected some of the questions are showing twice in the form view. The data looks fine in the table and the survey worked correctly.

Can you please provide more information, such as screenshots of where you are seeing the data twice in form view? Also are you using the web app and field app for data collection, and what do you mean by form view?

I am using both the web app and field app. Attached is a screenshot of the record when I open the data through my survey and the form view printout. As you can see some of the field show up twice. In the form view.
