Can you confirm you are using the latest version of Survey123 (3.11.123)? Are you able to share your form? If not, could you open an Esri Support case so that we can diagnose what is occurring?

Hi james, I've already upgraded to the last version and reboot my PC, then the problem was gone and the and the labels showed very well.
