ATC-20 Building Damage Assessment Midification Example

In case anyone else has any interest in using the Collector for ArcGIS Damage Assessment solution modification compatible with ATC-20 forms, I am uploading a doc to detail how to set up the schema that our organization has sitting on the shelf ready to go. We changed the symbol colors to match the 3 placard colors. It has only been tested in an exercise to date.  I welcome any constructive feedback how it can be improved.
