Citizen Service Request- Upload photo

I have downloaded the Citizen Service Request template file.

I have created the layers and published them.

I cannot seem to upload a photo. Either on a mobile device or on a computer. Do I need to add attachments?

When I click submit, the loader gif continuously stays on the screen.
