I have a feedback form which collects the information entered by user. I have requirements to include it in one of th story maps as a custom component. I am using Tabbed Story Map Series template.

Hi Aiad -- You can add your feedback form directly to one of the tabs of your Map Series -- when adding content choose the "Web Page" option and paste in the link to your survey form.
Depending on exactly how you want to do it, a couple ideas:

- present the story map in an iFrame and deal with the feedback form completely separately
