ArcGIS Monitor 2025.1 alerting on Requests Error Percent incorrectly

I have noticed several map services are getting flagged by monitor for 100% of requests error percent for sometimes 20 minutes at a time..  However, when looking at the logs for the service within ArcGIS Server Manager no logs exist to explain request errors.  I have 2025.1.1 for Agent and 2025.1 for Server.
Is there a known issue with this metric incorrectly reporting?

ArcGIS Monitor queries the ArcGIS Server usage reports to get metrics for the GIS Server. If you're seeing different statistic values in the ArcGIS Server logs, it suggests that the information in the usage reports are not consistent with the logs.

Please open an Esri Tech Support case so they can help you investigate this further.
