Geocoder Not working properly in ArcGIS for office

Hi all,

I am trying to create my own geocoder specifically for ArcGIS maps for office but it is not working properly for some reasons in ArcGIS maps for office.

I need "several columns to have zip, city, address. Instead that is what I have:

Does the geocode service work as expected in the ArcGIS Online map viewer if it is added as a utility service in your ArcGIS Online Organization?
