Add a Survey123 icon to preset widget icons

I have an editing widget in an Experience Builder app for viewing feature attributes and editing the STATUS field of a Survey123 AGOL feature layer.

I want to change the icon of the widget in Experience Builder to a Survey123 icon. Like this:

I'm aware that I can import custom icons. But I'm not a graphic designer, so I wouldn't know how to make my own icon cleanly. Could a Survey123 icon be added?
