What is the correct CSV format to map exact locations with values?

I'm having trouble finding the correct resource I need.

I have a CSV file with addresses, lat and long, and values. I don't know how to format the CSV correctly to map the exact points and capture the values. Or maybe I have to do that separately? I don't know. Please help.

You need to have a clean table with column headings. ArcGIS Online will try to guess the fields and type, but you can correct it when you upload it.
