We use S123 app version 3.19.120 on iOS. We have image questions where the constraint count-selected is more than 1. Then when wanting to remove one of the attachments... we click on the waffle icon that brings up thumb prints of the attachments in a pop-up like a Brady Bunch screen. We click on the applicable attachment to select it, so it is the one visible on the screen with its name and the count it is in the batch, i.e. 3 of 12. If the selected one is the last one, i.e. 12, it just deletes the last one, YAY - that is good/what we wanted. If we select one before the last one, it deletes the one selected, and the rest added after. i.e. if we selected number 3 of 12, it will delete attachments 3-12, leaving us with 2 attachments, when we expected 11.
