Features in mobile app display generic name after form update

I'm new to building web maps for Field Maps usage, so please bear with me if I get a few things wrong:

I have successfully added a feature layer to the map. When I first added this layer, each point feature was labeled with a name from one of the attribute fields.

After doing this, I went to the Field Maps Designer and added fields to the form. However, once I saved this change, the features were all renamed to "Points," like so:

How do I revert back to labeling the features with values from the attribute table? How can I avoid doing this again on future projects?
