Tile Cache Printing only level 0

I have a raster tile cache on our Portal that is only printing to the top level in the PDF or Image outputs from both the Web App Print Widget and the Web Map Print tool. And also in Pro.

I have tested the Mosaic dataset in Pro and the expected level of detail prints fine to PDF and JPEG.

However, the tile cache I created from this mosaic only prints the top level.

I cannot for the life of me understand why. There's no restriction on the export pixel size:

Is it available in both Web Mercator and Mercator projections?
