Just wondering if anyone has a template or example of a Geodatabase following the ESRI Address Data Model.

When you download the solution (http://solutions.arcgis.com/local-government/help/address-management/) it comes with a file geodatabase with example data. You can also check the Local Government Information Model data dictionary (http://solutions.arcgis.com/local-government/help/local-government-information-model/get-started/DataDictionary/DataDictionary.htm)
