survey123 form to arcgis desktop , metadata missing in table attribute table.

I did survey in survey123 form and exported shape file.

1. When i opened shape file in arcgis, the name(QUESTIONS)  in survey form is missing in attribute tables metadata . 

2. Drop down menu answers are missing in attribute table instead its showing choice 1 , choice2 . 

Please guide me on this.
