Has anyone already created a survey through Survey123 for small businesses to answer questions about changes in business practices during Covid-19

We are a community of small businesses. I have not used Survey123 in my work yet, so if someone already has a sharable survey created by someone specializing in business, we would really appreciate using it, if it is possible. I am looking for a survey that would ask questions in relation to change in hours, access to product (pick-up/drop-off, online, etc.), # of employees, minority/women-owned, among other relevant questions.

FYI, here's an example by Cobb County, GA

Cobb COVID-19 Restaurants Options 

Their site has a Survey123 survey for restaurants, and the results are shown in a dashboard.

Thank you for all of the information and links! Very helpful. I actually went ahead an made my first survey in Survey123! It may not be the most elegant solution but it may work for what we are needed.

I am trying to create a better ticket number system of sorts so business owners can better update their information over time. I'd appreciate any tips! 

(Also, we just received access to ArcGIS Pro. I am not super familiar with it yet, but will definitely explore the Business Continuity Solutions over the next few days)

Nice survey, Jessica. I like all the questions you added. Might adjust some of mine based on yours.
