Field Names in AppBuilder Widget

I created a web app from portal 10.3 using the Web AppBuilder. All out of the box so there is no custom configurations.  I am using the table widget in the app.  The problem is that the filed names are coming through as all lowercase letter.  I've double checked the attribute table in the mxd, and the map the web app is using and these both are properly capitalized in the table.  It seems this is only happening in the web app.

Any thoughts on a solution??
