Hi Lesley
This error can occur when the publishing process attempts to create a folder that already exists in your ArcGIS Online/Enterprise account.
If the existing folder in your account is empty and/or safe to remove, you could delete it and try publishing again.
Alternatively, you could try publishing this survey using a different name so that it creates a new folder.
