Using Survey123 with a feature layer from ArcGIS Online

I am creating a survey that uses a feature layer from ArcGIS online as a base.  I want to display information from the feature layer on my survey for information purposes.  This works fine if the field on the feature layer is one that you type your information in.  The field I am having trouble with is a drop-down in ArcGIS online.  In Survey123, it shows the code, not the label.  How do I get it to show the label, not the code?  I have attached screenshots.

Can you provide a copy of your xslxform so we can take a closer look? What I am interested to see if what is in your choice list sheet?

I re-created the survey, and this particular situation is fixed.  Thanks for getting back to me.
