I am attempting to replicate this paper form in Survey 123.

There are multiple choices (can be more than one) for each answer there is an associated $$$ value.

I want all selected choices $$$$ value totaled in the total charge- field to be added automatically.

I am not even sure how to assign a integer value to these choices with a text label in the multiple choice question type.

Should I even be using this question type or is there a better solution? see image below of what I am trying to replicate.

Hi Joe,

There are couple of ways to consider replicating the functionality form, though we can't replicate the exact layout.  The first would be to have a list of questions that have the charge; set the default to 0 and sum them up (essentially, rely on the fact that you charge them something means that it's "selected").  The second method would be to have a repeat section where you select the charge reason and charge and then sum that up; this would prevent you from having empty charge codes

I was doing this with multiple choice question type... but the functionality wasn't working. I created a case# : 01974565 and they are looking into it. may be a bug. it was a simple sum calculation.
