Survey123 3.24.21 Crashes When Taking Photo on iPhone 17 Pro Max (iOS 26.0)

Hi all,

I'm experiencing a consistent crash when attempting to take photos in Survey123 version 3.24.21 on an iPhone 17 Pro Max. I’ve done extensive testing across multiple devices and OS versions, and the issue appears to be specific to this hardware.

Issue Summary

• When I try to capture a photo using the camera inside Survey123, the app crashes immediately.

• This happens before the camera fully loads.

• The crash is repeatable across multiple surveys.

Other Notes

• Attaching a photo from the library works fine — only in-app camera capture causes the crash.

• Happens even in very simple surveys (e.g., one image question, no repeats or logic) and complex ones.

• Appears to be hardware-specific (iPhone 17 Pro Max) rather than OS-specific.

Questions

1. Has this issue been reported by other iPhone 17 Pro Max users?

2. Is Esri aware of compatibility problems with iOS 26.0 or the new camera hardware?

We just had a user call in with the same issue. IPhone 17 Pro Max, IOS 26. Survey123 closes when the user attempts to take a photo.

Same issue today (01Oct2025) with IPhone 17 Pro Max, IOS 26.

Survey opens, allows info to be submitted, user attempts to open photo action, and survey crashes. Reopen survey, allows to open where user left off, crashes again if photo is attempted.

App settings for Survey123 are verified to allow access to location, camera and cellular data.

We have found that upgrading the iPhone OS to 26.0.1 resolves the crash issue on the iPhone 17 Pro in Survey123.

Upon seeing your post, I just tested on my iPhone 17 Pro Max (which was upgraded to iOS 26.0.1), and I am still experiencing the same issue. I have even completely shutdown and restarted the phone. All apps closed (including Survey123). Survey123 still crashes upon trying to take a photo from the app.

Also you state iPhone 17 Pro (not Pro Max). All of the reports I have seen have been specifically the Pro Max, so not sure.

Our current work around is to take a photo using the photo app and then attach it to the survey.

I verified on our device iOS 26.0.1 on iPhone 17 ProMax that the app will still crash. 

Jonathan, you mention an iPhone 17 Pro...myself and Cameron are referencing an iPhone 17ProMax.

Thanks for sharing the update. The update iOS update resolved the issue on the iPhone 17 Pro, but not the iPhone 17 Pro Max. We are looking into this issue and will update this thread when we know more.
