Survey not showing in mobile app

I used the browser Survey123 to design and publish a survey. I shared it to a group, went to my mobile device, downloaded the survey, and this is what it looked like when I tried opening on my mobile device.

However, the browser was able to display it, and I was able to submit a filled-in survey. Has this happened to anyone else?
