Survey 123 fields not showing

I have been working on a survey form. I established it through make for a webhook. I submitted data through survey 123 but the QA / QC email shows up empty for some fields when I click on the form. However, it does show on survey 123 online, so only issue is with the QA / QC. Any ideas would be great. Thank you
