report quick reference does not contain all questions

1) why don't my report quick reference or sample feature report contain questions from my survey?
2) for select multiple questions can you format the check boxes in a report? i have seen checked boxes colored blue in many reports, in mine the are just white. can you format the check boxes somehow?
