Related table within a feature service

Helllo,

I'm creating a survey which is linked to an existing feature service - I have created a related table that links to the feature service. 
Within the survey itself i also have a repeat questions ( as per screenshot), however when i attempt to publish the survey the error message appears stating the table is not found?

Has anyone experienced this before?

Hi Craig,

What's the name of your related table ? 

The name of the related table and the name of the begin_repeat in the excel file must match. In your excel sheet the begin repeat is called : EquipmentCondition ( based on your screenshot )

Hi Craig,

In your excel sheet for the survey, you should put in the name column of the begin repeat, instead of EquipmentCondition, it should be PlayParkEquipment

Made those changes but I'm now faced with another error message!

Hello Craig,

i apologize to have misread your image, the begin repeat name must be the name of the related table, Play_Park_Equipment_TBL
