Likert Appearance Not Consistent in S123 in EB Widget

Hello All!,

I am having an issue with the likert appearance for one of my questions. It works fine in other places, but this one question it is not working (see photo). 

Any ideas?

the two fields have the same settings with the exception of the choice list.

Ok. I may have found the Issue--- the List has a choice filter. Any maybe the EB widget doesn't like that.

That is the only thing I can think of. I will just change the appearance and move foreword.
