Incorrect data showing in form view when looking at data in the data tab

Recently I have noticed that if I am reviewing completed surveys through the data tab for the survey, if I click on the point on the map it will show the correct data in the form view initially but then if I click any subsequent points the data in the form view is not showing the correct information for the point that I selected.  Attached is a screen shot showing a form view displaying the correct info for a point and a second form view displaying the attributes for a different point when I select the exact same point.

 Any suggestions as to why this might be happening?
