Getting Send Error - Layer 'layer' not found in feature service

I am currently struggling with a survey that I created with an existing feature service. The service has a point feature and a related table.  I was able to get it published through survey123 connect but whenever I try to submit something in the app it gives me a send error.

I have checked to make sure the feature service and the form id are the same. The feature point layer also has the same name. I'm not sure if that's causing issues. I'm not really sure what else could be wrong.

I'm using S123 connect V3.18.123 and S123 app V3.16.107

Most of Arc uses layer numbers so its ok to have dup names but not a great idea in general  But 123 for sure uses the name of the form and the repeat name to match up what layers to dump data into.  So good chance that is your issue.

I ended up changing the name of the service and remaking the survey to see what happens. I still get the same send error. I've contacted tech support to see if I can get an answer from them.
