Features do not show up as records in duplicated layer

I had a feature layer set up that had point locations, along with a few related tables for recording maintenance records. We also have a separate web app for visualizing records that was pointed at the REST service URL. It was working great until about a month ago, when the features stopped showing up in the rest service. They showed up fine in the Map Viewer and ArcGIS Pro, it was only the rest service that had the issue. There's a second hosted feature layer with the same structure and settings, which is still working fine. If anyone has an idea of how to fix that issue, it'd be great.

In the meantime, I exported the problematic hosted feature layer + the related tables as a FGDB and reuploaded it to AGOL. That version is working normally, with all the features showing up in the linked web app. I copy and pasted the XLSform from the old layer into the new one. That also seems to work ok for the most part. However, the issue is that the existing point features are not showing up in the inbox, and if I look at the Survey123 form in the web browser it shows 0 records. So it seems like the issue is that the existing point features are not being interpreted as survey records. Our normal field workflow is for field staff to select the location from the inbox, and add maintenance records as child records/repeats - so it's important for the inbox to be populated with those existing features.
