I used a sample template to create my feature report and then formatted the form and added the "if statement syntax" to each of the questions in my survey so that only questions with data would populate on the report .

I was able to successfully upload the template without any syntax errors but keep getting this error when trying to run a report with my template.

Hi @KristinaWood 

There is a known defect that produces this error.

BUG-000160736: Generating report in ArcGIS Survey123 returns an error message when the form_title and form_id in the survey’s .xlsx form start with an integer.

Could you please check the form_id field in Survey123 connect to see if your Form ID begins with an integer?

Thank you for your response. I checked the .xlsx form that I built using Survey123 connect. Under the settings tab I see that the form_title and form_id do not start with an integer. Is there anywhere else on the .xlsx that I should check or do you know of any other possibilities?
