Error 'critical' preventing survey123 connect being published

I am attempting to make a Survey123 connect form. I have completed the XLSform. I have completed a Bug Analysis – I get one info suggestion for a Note which I have attempted to fix within the settings- instance_name as the ersi help page suggests. However it does not register this.

My main problem however is that I cannot publish the survey I receive a blank 'critical' error. The 'creating folder', 'creating feature service', 'updating service details', 'creating feature layers and table' do get completed before the ! Critical message. 

If I then attempt to publish again I receive an error code 400 – file title not available. As suggested in other posts on this forum, if I delete the folder in my ARC GIS ‘My Content’ folder and remove the .itemInfo file from the ‘My Survey Design folder’ I go back to receiving the blank critical error message.

Could anyone please advise, is the instance_name issue the reason it wont publish or is there another issue please and solution you may be able to suggest.

I have a User Type: Professional Plus Role: Administrator ARC GIS account for information, but this is the first Survey123 Connect form I have created.

Have you tried enabling logging, in settings underneath the diagnostic tab.  That might help pinpoint the issue.  Also if you can, sharing the xlsx may help.

Thank you very much for you quick response! 

I have opened the diagnostics tab and it continuously is 'Searching for AppStudio consoles' should this be the case? 

XLSForm - it has lots of validation violations when I moved it to sheets so I could share it here.

You have to toggle on logging

The tell it where the log file should go.

Thank you for the clarification, I did this. But unfortunately it still is preventing me publishing with a blank Critical Error! 

Any other suggestions?

Did you look at the log file? If you can upload it, it would help. It should be in the location you set when you turned on logging.

Also, if you can, uploading the xlsx would help as well.

Not an expert on these log files, but there is an error here:

Had you previously published the survey successfully, made changes and then republished?  It appears to being trying to update feature service definition.  Have you tried doing a fresh publish, i.e.  save survey as a different name and then publish the new survey?  

I was able to publish the survey from the file you sent without error, with the caveat that it didn't include the images you have in your media folder.

Was this ever resolved? I am having the same issue and have tried creating and publishing a new survey but keep getting the blank critical error. When I analyze before publishing it says Critical: 0 which is confusing, because then publishing fails with no explanation. 

The original survey had already been published successfully and I just changed the order of a couple of the questions. No other changes. In fact, I can't even publish a blank survey as I get the same blank Critical message.
