Are we still restricted to using default 'Survey-SurveyName' folder or can we move the form?

Per this comment, is it still the recommendation to not rename the folder created in ArcGIS Online on publish, or move the survey form item?

Yes never move it or rename it.  There is a folder id in the .info file.  This is wrapped up in AGOL rules so it probably will not change.
