Adding ArcGIS Online map to survey

Hi, i've got problem with adding map from ArcGIS Online which i've published on my account.
This is my map in the application. Everything is right, looking good. This is what i wrote in .info file:

I tried everything, do you have any auggestions or ideas how to put my map into online form?
